School-Based Rehab Services (SBRS) Speech Therapy (SLP) Referrals
The Preschool Speech and Language (PSL) program provided by Quinte Children's Treatment Centre (QCTC) is offered until a child's entry to school. Following school entry, further speech support may be provided by the SBRS program, where required.
SBRS-SLP Referrals for Kindergarten Students (JK/SK Eligibility Screens)
The process for referral for JK and SK students is different than for students in Grade 1 and older.
For students in Kindergarten, QCTC SLPs may complete eligibility screens to support further referral to the SBRS Speech Therapy program, if required.
Our school (educator) teams, help to identify students in Kindergarten who demonstrate a potential need for targeted support. An assessment by the school-board SLP is not required in order to make a referral for this group.
Schools may refer students to QCTC when concerns are identified by completing the
available referral package for SBRS SLP which includes:
- SBRS SLP Kindergarten Eligibility Screen Referral Form
- Educator Checklist and Word List Screening Tool (English) or Educator Checklist and Word List Screening Tool (French)
SBRS-SLP Referrals for Grade 1 and Up
Students may be referred to SBRS SLP when speech needs are more significantly impacting their communication in the school environment.
Students in Grade 1 and above are first assessed by the school-board SLP who submits a referral for SBRS SLP. They complete the referral package which includes:
- SBRS SLP Referral Request Form
- Consent for Referral and Information Sharing
- SBRS SLP Referral Information Form
